Participant Liability Waiver and Assumption of Risk
By signing this waiver, I acknowledge that I am voluntarily participating in one or more activities, workshops, classes, wellness services, or events offered or sponsored by The Motherhood Village®. I understand that such programming may include, but is not limited to, physical movement, parent/child interactive exercises, wellness practices, and community events held either on-site at the facility or off-site at designated locations.
I understand and agree to the following:
· I voluntarily assume full and sole responsibility for any risks, injuries, or damages, known or unknown, that I, my child(ren), or any guest(s) I bring may sustain as a result of participation.
· I understand that while The Motherhood Village® promotes wellness and safety, some activities may involve inherent risks, including but not limited to: physical exertion, slips and falls, minor bumps or bruises, emotional responses, or unintentional contact with others.
· I release, waive, discharge, and covenant not to sue The Motherhood Village®, its owners, directors, employees, contractors, or volunteers for any liability, claim, or cause of action arising from injury or damages sustained during participation—except in cases of gross negligence or willful misconduct.
· I understand this waiver applies to all forms of participation, including classes, open play, support groups, wellness events, or special community gatherings, and that it remains in effect for the duration of my involvement.
· I agree to supervise my child(ren) or minor guests at all times, unless they are enrolled in a supervised activity as expressly defined by the program.
· I understand that it is my responsibility to disclose any relevant health conditions or special accommodations to program staff in advance of participation.
· I agree to indemnify and hold harmless The Motherhood Village® from any claims brought by third parties that arise from my or my child(ren)’s actions while on the premises or during programming.
· I acknowledge that I have received, read, and agree to abide by The Motherhood Village® Facility Code of Conduct. I understand that failure to comply with the Code of Conduct—including supervision of minors, respect for community safety, and behavioral standards—may result in the immediate suspension or revocation of access to services, classes, or the facility.
This release is binding upon me, my heirs, legal representatives, successors, and assigns.
